چکیده

To compare the prognostic effects of segmental ureterectomy (SU) and radical nephroureterectomy (RNU) for urothelial cancer of the ureter (UUC). Databases including Pubmed, Embase and Cochrane online library were selected for systematic review of retrospective trials that comparing outcomes of SU and RNU in the patients with UUC. Interested data including hazard radio (HR) for overall survival (OS), cancer specific survival (CSS), intravesical recurrence free survival (IRFS), recurrence free survival (RFS) and metastasis free survival (MFS) were extracted by two reviewers independently from selected trials. Meta-analysis was performed using RevMan 5.3 software. Eight eligible trials evaluating SU vs RNU for UUC were identified including 895 SU cases and 2149 RNU cases. The results showed no significant difference between the two kinds of surgical methods in HR for OS (P=0.86), CSS (P=0.23), IRFS (P=0.95), RFS (P=0.17) and MFS (P=0.23). Our analysis showed that SU had equivalent prognostic effect for UUC compared with RNU, which was considered the standard method. Because of the inherent limitations of the included studies, further large sample, multi-centric studies and randomized control trials should be undertaken to confirm our findings.

Kidney-Sparing Management Versus Nephroureterectomy for Upper Tract Urothelial Carcinoma: a Meta-Analysis

Upper tract urothelial carcinoma (UTUC) is rare and occupied 5% of the urothelial carcinoma (Munoz and Ellison, 2000; Siegel et al., 2013). The standard treatment of UTUC is nephroureterectomy (NU) with bladder cuff resection which means one kidney would be lost after this extirpative management. It yields high risk of followed chronic kidney disease and makes following pharmacotherapy treatmen...

Oncologic Outcomes of Kidney-sparing Surgery Versus Radical Nephroureterectomy for Upper Tract Urothelial Carcinoma: A Systematic Review by the EAU Non-muscle Invasive Bladder Cancer Guidelines Panel.

CONTEXT There is uncertainty regarding the oncologic effectiveness of kidney-sparing surgery (KSS) compared with radical nephroureterectomy (RNU) for upper tract urothelial carcinoma (UTUC). OBJECTIVE To systematically review the current literature comparing oncologic outcomes of KSS versus RNU for UTUC. EVIDENCE ACQUISITION A computerised bibliographic search of the Medline, Embase, and Co...

In this video Mello et al. (1), demonstrate the case of a Robot assisted segmental ureterectomy for a patient with a solitary kidney with high medical co-morbidities that had a single localized ureteral lesion consistent with a urothelial neoplasm. The surgical team carefully diagnosed and staged the ureteral tumor. A formal cystoscopy, bladder washing cytologies, retrograde pyelogram and urete...

The Characteristics of Recurrent Upper Tract Urothelial Carcinoma after Radical Nephroureterectomy without Bladder Cuff Excision

PURPOSE To investigate oncological outcomes based on bladder cuff excision (BCE) during radical nephroureterectomy (RNU) for upper tract urothelial carcinoma (UTUC) and to provide clinical evidence of tumor recurrence in patients without BCE. MATERIALS AND METHODS We retrospectively collected data of 372 consecutive patients who underwent RNU at our institution from May 1989 through October 2...
